AI medical scribe · the doctor signs every note

You talk to the patient.
Notera writes the note.

With the patient's consent, Notera listens to the consultation in Hindi, Telugu, English or any mix of them, and drafts the case note and prescription in your clinic's format. You read it, correct it and sign. Nothing is final until you do.

    How it works

Four steps, and the doctor owns the last one.

    1. 01 · Consent

The patient says yes, or Notera stays off

Before each consultation, the doctor or front desk asks. Notera shows the question in the patient's language and logs the answer with the note. If the patient says no, nothing is recorded.

    2. 02 · Listen

You talk the way you always do

A phone or a small mic on the desk. Notera follows Hindi, Telugu, English and mixes of them, and keeps doctor, patient and attendant apart.

    3. 03 · Draft

The note is waiting when you finish

End the consultation and the case note and prescription are drafted in your format. Every line links back to what was said, so you can check it.

    4. 04 · Review & sign

You correct it, approve it, sign it

Edit any line, change a dose, add a test. Only a note you have approved is printed or saved to the patient's record.

    What it writes

A complete OPD note, in your clinic's format.

Notera fills the same sections you would, in the order your clinic uses, with the shorthand your pharmacist already reads.

    Languages

Patients switch languages mid-sentence. So does Notera.

Consultations in Hyderabad rarely stay in one language. Notera follows Telugu, Hindi and English, and every mix of the three, then writes the note in clinical English, the way most Indian case records are kept.

Advice for the patient can be printed in Telugu or Hindi alongside the English note. Other Indian languages are set up and tested with your own consultations during a pilot.

    The clinic console

The day's queue, the live note, the signed prescription.

Front desk adds the patient. The doctor records with consent, reads the draft, signs it, and the prescription prints on your letterhead. Every signed note is in the patient's history.

    Privacy and safety

Built for the consulting room, not around it.

A scribe hears things patients tell only their doctor. These are defaults in Notera, not settings someone has to remember to switch on.

    1. 01

      Patient consent, every time

Consent is asked before each consultation, in the patient's language, and stored with the note. No consent, no recording; the doctor writes that note by hand or by typing.

    2. 02

      The doctor approves every note

A draft is marked as a draft until the doctor reads it and signs. Nothing prints, and nothing is saved to the patient's record, before that.

    3. 03

Handled per India's DPDP Act, 2023

Personal data is collected for a stated purpose, kept only as long as needed, and visible only to your clinic's authorised staff. Patients can ask for their data to be corrected or erased.

    4. 04

      Audio deleted after approval

The recording is deleted once the doctor approves the note, unless your clinic chooses to keep recordings, for a period you set.

    5. 05

      Not a diagnostic tool

Notera writes down what the doctor decided. It does not diagnose, suggest treatment or check doses. Clinical judgement stays with the doctor.

    FAQ

    Questions doctors ask us.

Yes. Consent is asked before each consultation and logged with the note. If the patient declines, Notera does not record, and the doctor writes that note as usual.

It can mishear, like any listener. That is why every note stays a draft until the doctor reads and signs it, and why medicine names, doses and durations are shown in their own table so they are easy to check.

No. It writes down what the doctor said and decided. It is a documentation tool, not a diagnostic or prescribing tool.

Telugu, Hindi and English, including switching between them in the same sentence. The note is written in English; patient advice can be printed in Telugu or Hindi. Other Indian languages can be set up during a pilot and tested on your own consultations first.

It is deleted once the doctor approves the note, unless your clinic chooses to keep recordings for a period you decide. Notes and personal data are handled per India's Digital Personal Data Protection Act, 2023.

A phone, tablet or laptop with a browser, and a microphone that can hear both sides of the desk. We check the room with you during setup.

Every signed note can be printed or saved as a PDF on day one. Connecting to hospital or clinic software is scoped with you on the Hospital plan.
